ACTAO highly recommends all ACTs complete the Court Audio Recording Feedback Survey - once submitted, goes directly to MAG, Court Reporting Services.
The survey is to be completed when there are issues with the quality of the recording and/or the annotations - both of which form the court record.
The Association has requested that MAG provide a confirmation email and/or confirmation number to ACTs once they have completed and submitted the Audio Recording Feedback survey and a copy of the survey the ACT completed be sent to the ACT.

Please also note Justice Templeton's addendum on the last page of R. v. Vandergust, which may also be brought to the attention of RMC and Supervisors when the required information has not been made available to ACTs which is required for the expeditious, accurate production of transcripts.

The Recording and Annotation Deficiency Report, is no longer used. It was created by the Association to be used by ACTs to report recording and/or annotation issues. Shortly after this form was created by the Association, MAG put in place the Court Audio Recording Feedback Survey. The Recording and Annotation Deficiency Report was to be emailed to the courthouse location where the recording/annotations were obtained to the attention of the RMC, Supervisor and/or Manager of Court Operations. The report may also be emailed to [email protected].
